Thyroid Optimization

The engine of your metabolism.

Your thyroid controls metabolism, energy, mood, and body temperature. Even 'normal' thyroid levels can leave you feeling terrible if they're not optimal for you. We go beyond basic TSH testing to get the full picture.

What is thyroid optimization?

Thyroid optimization involves comprehensive testing beyond the standard TSH check that most doctors order. We look at Free T3, Free T4, Reverse T3, thyroid antibodies, and TSH together. If your levels aren't optimal, we can prescribe compounded T3/T4 formulations tailored to your exact needs.

Common signs and symptoms

Fatigue despite adequate sleepCold hands and feetConstipationUnexplained weight gainDry skin and brittle nailsHair loss or thinningBrain fog and poor memoryDepression or low moodMuscle weaknessPuffy face or swelling

Treatment options

Compounded T3/T4 combination therapy
Desiccated thyroid (NDT)
Liothyronine (T3) supplementation
Selenium and zinc optimization
Iodine assessment
Autoimmune thyroid management
Adrenal support if needed

How the process works

01

Complete thyroid panel

We order TSH, Free T3, Free T4, Reverse T3, TPO antibodies, and thyroglobulin antibodies — the full picture, not just TSH.

02

Optimal range assessment

Your provider evaluates your results against optimal ranges (not just 'normal'). Many patients with 'normal' TSH have suboptimal Free T3 levels.

03

Compounded formulation

If treatment is needed, your provider prescribes a T3/T4 combination compounded to your exact dose — not a one-size-fits-all tablet.

04

Fine-tuning

Follow-up labs at 6-8 weeks to adjust dosing. Thyroid optimization is iterative — we dial in your perfect dose based on how you feel and your lab values.

Frequently asked questions

Why can't my regular doctor help with this?

Most doctors only test TSH and consider anything in the 'normal' range as fine. We test a complete panel and optimize to where you feel your best — which is often different from the broad 'normal' range.

What's the difference between T3 and T4?

T4 is the storage form of thyroid hormone that your body converts to T3, the active form. Some people don't convert efficiently, leading to symptoms even with 'normal' T4 and TSH levels. That's why we test and treat both.

Can compounded thyroid medication replace Synthroid?

For many patients, compounded T3/T4 provides better symptom relief than T4-only medications like Synthroid. Your provider will discuss whether switching makes sense for your situation.

Is Hashimoto's thyroiditis treatable?

While there's no cure for Hashimoto's, we can optimize your thyroid levels and manage symptoms effectively. We also address the autoimmune component through lifestyle and supplementation strategies.
